Transitional Care Hospitals

Kindred Transitional Care Hospitals (certified as long-term acute care hospitals) provide aggressive, specialized long-term acute care (LTAC) to medically complex patients who require extended recovery time. Our hospitals are unique in their ability to care for difficult to treat, chronically critically ill patients who require specialized and aggressive goal-directed care over an extended recovery period. Typical patients have multiple co-morbidities, multi organ system failure, and significant loss of independence, most following a traditional hospital stay.

Transitional care hospitals are licensed as acute care hospitals with additional Medicare certification that supports a length of stay measured in weeks (more than 25 days on average for Medicare patients) as compared to the typical five day stay for patients in traditional hospitals.

Our specialized services and extended care are tailored to medically complex patients who are unable to recover completely in a short-term setting.

We are proud of our clinical outcomes - in 2011, Kindred’s Transitional Care Hospitals treated the sickest, most medically complex patients, discharging nearly 70% of patients home or to a lower level of care after an average length of stay of 28 days. From 2008 to 2011, we have reduced rehospitalization rates by more than 8%.

Our hospitals have 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating or procedure rooms, high acuity units, special care or intensive care units (ICUs) and telemetry units. Transitional care hospitals are licensed as acute care hospitals and are accredited by The Joint Commission.
