COMPASSIONATE, TRANSITIONAL CARE
A subacute unit generally includes two types of patients - those who are receiving short-term rehabilitation to recover from an injury or illness, and those with multiple medical problems or illnesses who are receiving care for a longer period of time.
Patients in a Subacute Unit at a Kindred Hospital receive rehabilitation therapy that acts as a bridge, transitioning patients from hospital care back to the community. The focus is on physical and functional improvement and personal independence. A team of professionals works with each patient to restore health and help him or her achieve maximum independence in activities of daily living. Patients in our Subacute Care Units wear street clothes and can enjoy meals and activities in our dining/recreation room.
Discharge planning begins upon admission focusing on the individualized goal and desired outcomes. Our team members' objective is to help each patient reach his or her maximum health and functional ability which includes educating both the patient and their family members to successfully manage their daily routine after discharge.
A Team Approach to Caring
Upon admission and under the direction of a physician, patients are evaluated by our interdisciplinary team comprised of:
- physicians
- nurses
- case management and social workers
- rehabilitation therapists (physical, occupational and speech)
- recreational activities coordinator
- dietitian
- respiratory therapy team
- pharmacist
Our Subacute Services Include:
- comprehensive rehabilitation program (physical, occupational and speech therapies)
- pulmonary and respiratory care (ventilator and trach)
- advanced wound care
- IV antibiotic therapy
- pain management
- cardiac care
- dialysis
- post-surgical care
Managing quality and treatment results is central to Kindred's subacute programs. We monitor our residents' outcomes, including such quality measures as healing and medical improvement, functional gains, length of stay and return rates to the community.
