Jack began to experience difficulty breathing and had to go to the hospital where he was diagnosed with complications due to pneumonia. He began to receive antibiotic and respiratory treatments and when he was able to be transferred he was brought to Kindred Hospital Baldwin Park.

When he first arrived Jack needed complete assistance from his caregivers for all his needs. His team of respiratory therapists oversaw his pulmonary recovery, while his physical and occupational therapists helped him regain his mobility and independence over the activities of daily life. Jack made steady progress every day and, in less than a month after first coming to Kindred, was able to get up from bed, stand and walk with the aid of a walker.
Although Jack was challenged during his recovery with the task of getting back his fine motor skills and many other abilities that are easily taken for granted, he persevered with constant encouragement from his rehab team. At the time of his discharge just three weeks after arrival, Jack expressed that he was looking forward to continuing on his journey to recovery and shared some kind words with his caregivers: “I am very grateful to the staff and the rehab team for helping me achieve my goal – I’m going home.”

Luis Gets Back on Track
Luis, 46, was admitted to Kindred Hospital Baldwin Park from Presbyterian Hospital for bacterial meningitis, metabolic encephalopathy, and respiratory insufficiency.

At time of admit, Luis was medically fragile, breathing with ventilator support, and in an altered state of consciousness. He had gone through septic shock with periods of severe confusion, and initially had to be restrained to his bed for his own safety.
Prior to this recent acute medical episode, Luis was living at home with his sister. At time of discharge from Baldwin Park, Luis was weaned off the ventilator and decannulated, able to ambulate more than 200 feet with a walker, able to eat and speak, and able to tolerate out-of-bed activities without respiratory distress.
After only a couple of months, Luis was safely discharged to a rehabilitation facility. He and his family were extremely pleased with the expert care he received at Kindred Hospital Baldwin Park, and grateful to them for helping Luis get his life back on track.
Vivian Returns Home

Vivian is a 78-year-old female who was admitted from Queen of the Valley following multiple surgeries for metastatic cancer. She also presented with a colostomy infection and required a multi-disciplinary team approach to treat her complicated medical condition.

She was living independently at home prior to these medical events. Her medical history includes metastatic lesion, hysterectomy, partial cystectomy, and bladder reconstruction.
When she discharged home from Baldwin Park with her daughter and home health services, Vivian was clear of her infection, able to transfer with independence, able to ambulate more than 200 feet, and able to tolerate out-of-bed activities independently.
