Our Subacute Care Unit

 

Subacute care is specially designed to provide short-term rehabilitation to help you recover from an injury or illness, or to provide the compassionate care you need when facing multiple medical problems or illnesses for a longer period of time.

Our subacute care units are unique because they are located within our hospitals, ensuring that you have access to the clinical services and rehabilitation you need seven days a week. The focus is on your physical and functional improvement and independence. Our team of experts works with you to restore health and help achieve maximum independence in your daily activities.

We begin planning for your next steps upon admission, focusing on your individualized goals and desired outcomes. Our team provides education to both you and your loved ones on how to successfully manage a daily routine after you leave our care.

Our Subacute Units include services such as: 

  • A comprehensive rehabilitation program (physical, occupational and speech therapies)
  • Pulmonary and respiratory care (ventilator and trach)
  • Advanced wound care
  • IV antibiotic therapy
  • Pain management
  • Cardiac care
  • Dialysis
  • Post-surgical care

Under the direction of a physician, you receive care from our interdisciplinary team, which may include:

  • Physicians
  • Nurses
  • Case management and social workers
  • Rehabilitation therapists (physical, occupational and speech)
  • Recreational activities coordinator
  • Dietitian
  • Respiratory therapy team
