Shirley's Recovery from Respiratory Distress

Shirley was taken to the hospital after falling out of bed and showing signs of altered mental status.
After a stroke was ruled out, Shirley was admitted to the medical unit with metabolic encephalopathy. She was later transferred to the MICU for hypothermia and hypotension, and then to neurocritical care after experiencing episodes of hyperactive delirium. She developed hypoxia, with concern for flash pulmonary edema and possible aspiration pneumonia. Nearly a week later, her condition worsened, requiring intubation and transfer to the ICU. A bronchoscopy was performed, and PCR testing was positive for MRSA and rhinovirus. ENT was consulted for a tracheostomy, a feeding tube was placed, and her trach was later exchanged for a distal XLT.
Shirley was transferred to Kindred Hospital East New Jersey on full ventilator support for specialized respiratory care.
As part of Kindred’s weaning protocol, Shirley successfully tolerated 72 hours on a trach collar. A speaking valve was ordered, and her airway clearance was monitored. Staff then supported Shirley through capping trials, diagnostic procedures in which a cap blocks the opening of the tracheostomy tube. This allows patients to breathe and swallow through their upper airway so clinicians can determine whether the tracheostomy tube can be safely removed. After five days of capping, Shirley was successfully decannulated.
To celebrate this milestone, staff made a special announcement over the PA system and pressed the “Cheers” button in Shirley’s honor. After 29 days at Kindred Hospital East New Jersey, Shirley was discharged to a skilled nursing facility.
